Instructions for Weekly Log/Time Sheet

At the end of each week, reflect on what you have accomplished, how the week went, and what you would like to accomplish the next week to advance your learning to meet your goals. Here are a few key things to remember:

  • Reflection requires a little time; Weekly Logs that contain little to no information will be returned to redo them.
  • The Weekly Log is a way to communicate your learning needs to your supervisor – this can frequently spark a conversation/negotiation of activities to reach your goals.
  • The Weekly Log allows both you and your supervisor to provide constructive feedback for each other.
  • The Log also includes your timesheet. Use the ‘notes’ box to explain variance in work hours as needed (e.g. work-time off-site). Commuting time to and from your site is NOT included in your ‘work time’.

I am not requiring your supervisor to complete their section; however, I strongly suggest you share your self-reflection with your supervisor to receive their feedback for growth, improvement, advancing goals, etc.

Weekly Log/Time Sheet

Trick: This is a fillable Word document that you complete on your computer. Save it as a new document, naming the document with your last name, week # before you try to upload to ‘Assignments’ in Canvas.

Once the Log/Time Sheet is complete submit weekly by uploading it through Assignments in Canvas. Each document will be ‘saved’ over the previous week.

DO NOT:

  • Send a picture file of the document.
  • Print and try to complete by hand – the text boxes will not expand and one-line reflections are considered incomplete.
